4/4/2024 EnchantaVaughn Approved Behavior: Client presents good well-being and appearance. Client displayed full participation in session and contributed to discussion. Client displays an increased pattern of independence. Client was observed paying attention and asking appropriate questions. Client seemed to be enjoying the session, as evidenced by her interaction and attentiveness during activity. Client appeared to grasp understanding of accepting responsibilities on her own and reducing symptoms of ADHD. Mother reports client continues to struggle with making positive decisions at which these behaviors last at least 3 minutes in duration. Behavior: Client was provided with learning opportunities for self-determination skills. The client was requested to identify areas in which she would like to improve (e.g. interpersonal relationships, objectives, hobbies). Client was supported for displaying active listening skills and attending to her work. Client was attentive and open with counselor about her feelings and how to identify them. Client shared that she believes she will be able to master her feelings soon and control her anxiety with more practice. Client shared that she enjoyed the session she had with her counselor. Client mother reports she continues to struggle with building healthy relationships with others within the hone and community setting at which these behaviors last at least 3-4 minutes in duration on a daily basis. Intervention: QMHP provided guidance on alternative behaviors and clear instruction on the alternative behavior. QMHP provided positive feedback when client presented favorable actions expected in the home. QMHP explored positive behavior options with client to support her willingness to make good decisions and have positive behaviors at home, in the program, and in the community. QMHP commended client on her positive behaviors. Intervention: QMHP encouraged client to maintain her behavior and told client that QMHP was proud of her. QMHP offered simple praises to client for exhibiting the appropriate behaviors. QMHP continued to establish a positive supportive relationship with client. QMHP offered simple suggestions to the client for behaving appropriately in the home and community and encouraged client to practice the positive behaviors in session too. Response: Client acknowledges expectations and reviews goals and objectives with counselor and parent at the beginning of each session. Client utilized deep breathing techniques to relax and stay calm during stressful environment, especially when it’s a lot going on around her. Client was happy to go to a quieter space to finish the educational assignment, verbalizing appropriate understanding by asking and answering appropriate questions with QMHP. Response: Client displayed self determination to stay focused and pay attention while QMHP went over each prompt. Client was pleasant and cooperative and demonstrated a willingness to discuss the factors contributing to her stress and anxiety. Client was responsive to the interventions and attempted to implement skills right away. Client said she felt great and would continue to follow her treatment daily and adhere to her objectives.
